This is a note for your appointment, not a test. Rate the same symptoms every couple of weeks to see what changes, especially after starting or changing treatment. HRT, CBT and other options can all help.
Symptoms affecting work, sleep or relationships are a good reason to book a GP appointment. Bleeding after 12 months without a period, or unexpected bleeding after the first 6 months of HRT, needs a GP visit soon.

Each symptom scores 0 for none up to 3 for severe, for a total out of 24. Moderate hot flushes and poor sleep with mild low mood scores 5, with two symptoms flagged as bothering you a lot. It is a note for your appointment, not a test.

What NICE says to ask about
For your two worst symptoms we add what NICE NG23 suggests discussing: HRT for flushes and sweats, with menopause CBT alongside or instead; CBT for sleep broken by night sweats; HRT or CBT for low mood that started with other symptoms; vaginal oestrogen for dryness, which is fine alongside HRT; and testosterone if low desire has not improved on HRT. Add last time’s total to see the change. NICE reviews treatment at 3 months, then once a year, and bleeding is common in the first 6 months of HRT. Bleeding after that, or after 12 months without a period, needs a GP visit.
Perimenopause or menopause?
How NICE says it is recognised
Over 45, NICE NG23 says no blood test is needed: perimenopause is recognised from symptoms with irregular periods, and menopause after 12 months without a period. If contraception, a coil or a hysterectomy hides your periods, it can be judged from symptoms. Between 40 and 45 your GP may check a hormone called FSH; before 40, symptoms can be premature ovarian insufficiency, which needs a GP visit and blood tests. At 49 with irregular periods, that points to perimenopause, and HRT or CBT can be discussed now.
